It's rather simple if you're not over the port limit like it seems the case. Take a USB 2 device (like an USB Bluetooth/mouse dongle, old USB Stick, USB Keyboard etc) and a USB 3 device (most USB sticks) and stick each in every port. Simply select the right port type.
After all ports are set correctly click on the export icon (the right most below the list). This usually generates some SSDTs and an USBPorts.kext. You don't need the SSDT-UIAC, just put the kext into your boot loader.
